Pyramids containing the corticospinal tract are on the ventral surface of the medulla. At the medulla spinal cord junction, the pyramids turn sharply dorsal with 85-90% of the fibers crossing to the contralateral side and shift dorso lateral to enter the lateral funiculus of the spinal cord. Note that the ventral horns are pushed laterally as the corticospinal fibers pass between them as they move dorsally.

The descending trigeminal tract and nuclei are seen dorsal laterally. In the region of the dosal funiculus containing the gracile and cuneate fasciculi are the lowest parts of the gracile and cuneate nuclei.
